New Patients

Criteria
To register as an NHS patient at Shefa Medical Practice you should fit the following criteria:

1. Live within the catchment area (See below map of our catchment area and postcode checker)
2. Have United Kingdom Resident Status or equivalent.
3. Accept in full the terms of the Practice-Patient Charter with regard to the responsibilities of patients
4. Provide all necessary documentation to complete the registration process (see below)

Registration
To go ahead and register you must complete the following documents and bring them in to reception.

1. New Patient Registration Form
2. Family Doctor Service Registration Form
